These
are forms that we need filled out for each player on the
team. These forms will be distributed early in the season
to all players on the "A" Rep team but blank forms will
be kept here for future reference/downloads. Any players
called up to play games with our team will need fill out
these forms also.
Player Contact Information. This contains general
contact information and a release form that allows us
to use team photos on the website, etc.
To be used in the event of an emergency. This form will
accompany the team to all games and will be kept with
the Team First Aid Kit.
Nov-Jan Training Registration. This needs to be filled
out and submitted to the Soccer Office at RIM along
with $100 to cover the cost of the field rental at
Bechtel.
